PROBLEM TO BE SOLVED: To provide a personal information communication sheet which enables confirmation of a fact that concealed information is read secretly by a malicious third person, and a sealed document using the same. SOLUTION: The personal information communication sheet consists of first and second paper pieces which are connected through the intermediary of a folding part and have almost the same size. An address information mentioning part is provided in either the first or second paper piece on one side of the sheet, while a personal information entering space wherein information peculiar to a person is entered is provided in an area except the three peripheral edges of the first or second paper piece on the other side of the sheet, and an almost U-shaped adhesive part and release paper covering the adhesive part are releasably provided along the three peripheral edges except the connecting side of the first or second paper piece wherein the personal information entering space is not provided.
