PROBLEM TO BE SOLVED: To provide a perpendicular recording patterned disk medium exactly aligned with patterns on both front and rear surfaces and a magnetic disk drive loaded with the medium. SOLUTION: The patterned disk medium 1 is provided with data area patterns 12 and 22 respectively formed in advance on first and second surfaces SA and SB of a disk-shaped flat substrate having the first surface SA and the second surface SB of the side opposed to the first surface, servo region patterns 11 and 21 formed in advance like an arc which divide the data area patterns 12 and 22 of the first and second surfaces SA and SB respectively in a circumferential direction, and positioning marks PMa and PMb used for positioning in simultaneously forming in advance the servo area patterns 11 and 21 and the data area patterns 12 and 22 in approximately the aligned positions of the first and second surfaces SA and SB of the substrate.
