PROBLEM TO BE SOLVED: To provide a delivery packing medium set capable of matching the customer data described on a receipt or the like with the customer data described in advance on an application card packed together with the contents (a gift catalog or the like). SOLUTION: The delivery packing medium set has a receipt, an application card, a mount and a packing envelope to pack these items together with the contents. The packing envelope has a receipt take-out port to take out the packed receipt, the receipt is attached to the mount so as to be taken out of the receipt take-out port, the application card is formed in a predetermined area integrated with the mount, at least the address and the name and address of the customer are printed thereon, and the positional relationship between the mount and the packing envelope is maintained when packed.
