PROBLEM TO BE SOLVED: To identify even a form not having a ruled line without producing a dedicated form. SOLUTION: A corresponding point detection means 4 detects a corresponding point of a registration image and a form image of a processing target, and a conversion coefficient calculation means 5 finds a coefficient for converting corresponding coordinate values. A differential degree calculation means 6 finds a differential degree between the images on the basis of the conversion coefficient, and a decision means 7 outputs the registration form image having a smallest differential degree as the corresponding form.
