PROBLEM TO BE SOLVED: To provide a unified type application form with such advantages that its inventory management cost can be reduced and its overall workability ranging from an application phase to an examination phase is excellent. SOLUTION: Necessary items are entered in a common entry column 21 for all card issuing companies of an entry ledger paper 20 and an entry column 31a of an application form (e.g. the card application form 31 classified by company) selected from among card application forms 31 to 34 classified by company under the category of a card application form 30. Next, the card application form 31 is separated in a separation line 35 and released from its release paper 30c, and pasted to a pasting column 22 of the entry ledger paper 20. After that, the entry ledger paper 20 and the card application form 31 are combined together in one piece and the combined form is certified as genuine by affixing a seal 51 in tally seal columns (a) and (b) of the entry ledger paper 20 and the card application form 31.
