PROBLEM TO BE SOLVED: To provide an image forming apparatus capable of attaining user authentication on the basis of information signified by a symbol described in a particular region of an original and further managing data extracted from the information and an operating state of the apparatus under the user authentication control. SOLUTION: A CPU 10 collates information signified by the symbol described in the particular region 205 of a registration original 200 registered in advance with information signified by the symbol descried in the particular region 205 of the original 210 captured newly by an original capture section 12 in the case of copying the original and the image forming apparatus is configured that when coincidence of the both is decided, copying of the original is permitted, and when the copying is permitted, a history memory 173 stores an operating history such as data extracted from the information and charging information in response to the use.
