PROBLEM TO BE SOLVED: To provide a two-dimensional code having embedded information (watermark information) in addition to general information, a device and method for forming the two-dimensional code, and a device and method for reading the two-dimensional code.

SOLUTION: This two-dimensional code 1 comprises unit cells (cells 2 and 3) of contrast patterns arranged in a matrix. The unit cell includes a reference color cell 3 and a plurality of discolored cells 3' of a tone different from the reference color. The two-dimensional code 1 shows general information by the arrangement of cells 2, 3 and 3' of bright color and dark color, and embedded information by the arrangement of a plurality of discolored cells 3'.
