<P>PROBLEM TO BE SOLVED: To provide a questionnaire system promoting application to a questionnaire from a magazine purchaser for collecting questionnaire information from the magazine purchaser. <P>SOLUTION: In a magazine, an URL of a questionnaire destination and an identification code for questionnaire application are indicated. A communication terminal is provided with a means for inputting the URL and the identification code and a means for accessing a questionnaire management server based on the inputted URL and transmitting the identification code for application to the questionnaire. The questionnaire management server is provided with an identification code database storing an identification code for application to questionnaire and usage condition information, a collation means collating the identification code to the identification code registered in the identification code database in receipt of the identification code from a communication terminal, an identification code usage condition determination means determining whether the matching identification code is already used or not, and a control means performing processing while determining the questionnaire application to be effective when the identification code is determined to be an unused one. <P>COPYRIGHT: (C)2006,JPO&NCIPI |
