PROBLEM TO BE SOLVED: To prevent troubles on transferring such as meandering and plugging of paper in a single piece printer from occurring in a slip form provided with a label function and an envelope function. SOLUTION: A label part 13 releasably bonded with a pressure-sensitive adhesive 22 applied on the back face of an upper paper 21 and a releasing agent 32 applied on the surface of an lower paper 31, is provided on a sheet body 11 which is a laminate of the upper paper 21 and the lower paper 31 both with the same size. In the vicinity of this label part 13, three sides on the peripheral edge parts of the upper paper 21 and the lower paper 31 are stuck with the pressure-sensitive adhesive 22 applied on the back face of the upper paper 21 and the remaining one side is opened to provide an envelope part 14. It is possible thereby that no step difference is made on either the surface or the back face of the sheet body 11, and no difference in thickness is generated on the whole sheet body 11.
