A pharmaceutical preparation comprising levosimendan or a pharmaceutically acceptable salt thereof as a therapeutically active ingredient together with one or more pharmaceutically acceptable excipients, which is in the form for intraoral or intranasal transmucosal administration. Use of levosimendan or a pharmaceutically acceptable salt thereof for the manufacture of medicament for transmucosal administration of levosimendan or a pharmaceutically acceptable salt thereof to oral or nasal mucosa. The medicament is suitable for treating heart failure.
