<P>PROBLEM TO BE SOLVED: To efficiently collect information of a malfunction that user concerns about a device such as a combination machine by a management center. <P>SOLUTION: When there is a designation by a user or it becomes a timing of periodic notification, a control section 210 acquires a log of errors accumulated on an error registration buffer 222 and information of a component condition such as a residual amount of toner stored in a component condition information memory section 224 and forms a check list having check items of malfunctions estimated by the errors or component conditions to display it on a touch panel 230. A user may select an item in the check list that the user concerns. The information of the check item selected by the selection operation is transmitted to the management center 10.
