PROBLEM TO BE SOLVED: To provide an inexpensive delivery slip with proper workability, which can simply and surely prevent accompanying documents such as product specifications from coming off during delivery or prevent these documents from being discarded without being taken out at a destination. SOLUTION: In this 1 pt type delivery slip 10, a slip area 12 of a printing base material 11, and tack paper 14 are releasably bonded together by a releasing structure; the printing base material 11 is equipped with the slip area 12 and the product specifications 13; and the tack paper 14 is a sheet wherein a tack base material, a pressure-sensitive adhesive and release paper are laminated in this order. A peripheral edge of the release paper is peeled in a U-shape via a half cut hc1, and the delivery slip 10 is stuck on a pack. In this case, a bag-shaped storage part for housing the product specifications 13 is formed between a central part of the release paper and the pack; the product specifications 13 are enclosed in the storage part; and after that, an upper end of the release paper is peeled via a half cut hc2, so that the storage part can be sealed.
