<p><P>PROBLEM TO BE SOLVED: To provide an application form creation system that is configured inexpensively with a general purpose machine and increases user friendliness owing to its easy operation. <P>SOLUTION: The application form creation system 1 has: an OCR 6; an operating terminal 2 comprising a keyboard 12, a mouse 13, a monitor 14 and a PC body 16; a host 4 having a database D storing data on issued driver's licenses; and a printer 8. When an applicant enters a driver's license number via the operating terminal 2, the database in the host 4 is searched with the number as the key to extract corresponding driver's license information.
