A preventive and/or therapeutic agent for diseases accompanied by a chronic muscle/skeleton pain, which contains as an active ingredient either a compou nd having antagonistic activity against an adenosine A2A receptor, such as, e.g ., a xanthine derivative represented by the following formula (I) (wherein R1, R2, and R3 are the same or different and each represents hydrogen, lower alkyl, lower alkenyl, or lower alkynyl; R4 represents cycloalkyl, -(CH2)n-R5 , or the following formula (II); and X1 and X2 are the same or different and each represents oxygen or sulfur), or a pharmaceutically acceptable salt of the compound.
