Five almost flat-circular-shaped pattern electrodes (312A1-312A5) corresponding to the distribution shape of coma aberration are dividedly formed, adjacently, in parallel and in the longitudinal and radial direction and in the direction perpendicular to a radial direction in which an object lens moves, on at least one of transparent electrodes (312A) respectively formed on the inner surfaces of a pair of transparent glass substrates. A voltage affording a phase difference serving as a reverse-polarity coma aberration according to coma aberration occurring on a pupil plane is applied to pattern electrodes (312A2, 312A4) positioned on the opposite sides of a pattern electrode (312A3) corresponding to the center of an object lens pupil diameter (341) to change a refractive index. Accordingly, a high-accuracy coma aberration correction in conformity with the movement of the object lens is possible with a simple pattern division.
