<P>PROBLEM TO BE SOLVED: To enable an applicant to store duplicate served both as an application form for admission and a maximum limit contract document filled in at the time of application and to easily confirm the contract contents later. <P>SOLUTION: Machine read information and password unique to a card are accepted to the applicant by telephone by a method using the card having the unique machine read information for every card, the application form with card on which a set of paper sheets served both as the application form for admission and the maximum limit contract document are integrally formed, and data is inputted.
