A medical lead (50) for electrical stimulation or sensing. The medical lead (50) has a generally flat paddle (62) on the distal end of the lead body (52). An electrode array (64) is provided on the paddle (62). The electrode array (64) has directional electrical field properties relative to the first and major surfaces of the paddle (62) (e.g., the electrodes (66) are exposed through only one of the major surfaces). An orientation marker (67) for determining orientation of the lead (50), with the orientation marking (67) including fluoroscopically viewable material. The orientation marking (67) is displaced from the longitudinal center line of the paddle (62) or is arranged (or dispersed) in a asymmetric manner with respect to the width of the paddle (62).
