PROBLEM TO BE SOLVED: To provide a low-cost slip storage body with a card having a smart shape, in which slips such as a form or the like and cards such as a driver's card or the like can be properly distributed. SOLUTION: The slip storage body 10 with a card comprises a first sheet substrate 11, a second sheet substrate 12, a third sheet substrate 13, a card part 14 and a form 20 or the like. The half left side of the first sheet substrate 11 and the second sheet substrate 12, which are laminated together at three sides with a pattern paste a, is a substrate for forming a bag-shaped pocket for storing forms. The half right side of the first sheet substrate 11 and the third sheet substrate 13, which are laminated together in substantially all surfaces with an adhesive layer b, is a substrate for forming a laminated structure. A white color tone colored layer is provided to the first sheet substrate 11, a substrate having transparency is used for the second sheet substrate 12, the card part 14 is formed so as to be released and peeled off substantially in a card shape, having information associated with the form 20.
