PROBLEM TO BE SOLVED: To provide an application form which has a simple constitution and in which an action on a declaration of an intention of a consent to the detail of a contract can be easily performed. SOLUTION: The application form 1 is prepared by binding a plurality of papers 2, 3, 4 and 6 on which various information such as explanatory sentences and greeting sentences are described and which are bound with each other, separably with a payment sheet 5 which is folded by making payment information entry columns inside and is made closed on the opposite side end to a perforation 8 for both folding and cutting. An action that separates the payment sheet 5 and makes it to a double-page spread condition, is regarded as the action on the declaration of the intention of the consent, and a payment of a predetermined charge is considered as an application of the contract.
