PROBLEM TO BE SOLVED: To provide an appearance inspection device capable of improving inspection accuracy and simplifying setting of an optical system, and a PTP packaging machine equipped with the appearance inspection device. SOLUTION: This appearance inspection device 12 is equipped with an illumination means 21 for irradiating a PTP film equipped with a tablet and a cover film to which discrimination information (a sealed part and a printed part) having transparency to infrared light is attached, with the infrared light from the pocket part side, a CCD camera 22 having sensitivity to the infrared light, and an image processing device 23 for processing an image signal from the CCD camera 22. In the image processing device 23, first of all, binary image data are generated by the first binarization means 29 in disregard of the sealed part on the tablet, and a tablet region is specified, and a foreign substance in the tablet region is determined. Then, binary image data are generated by the second binarization means 31 in disregard of the printed part on the cover film, and a foreign substance on the cover film is determined.
