PROBLEM TO BE SOLVED: To provide an electronic medical record input system capable of efficiently performing the work of inputting results of Hess Tests into electronic medical records and supporting diagnoses based on the results of the tests.

SOLUTION: When a plurality of indicated points as results of the Hess Tests are inputted into an input template screen 1000 in a prescribed sequence of input, a control means 11 obtains the coordinates of the indicated points inputted, computes straight lines connecting the indicated points associated with each other by the indicated points association information 600, namely the lengths and inclinations of the straight lines forming a Hess diagram and displays the Hess diagram on the input template screen 1000. The control means 11 selects the name of a disease corresponding to the coordinates of the indicated points and the lengths and inclinations of the straight lines from a monocular disease data 700 and binocular disease data 800 and displays the selected name of the disease on a monitor 13.
