The use of sildenafil in the manufacture of a medicament for treating or preventing pulmonary hypertension selected from primary pulmonary hypertension and secondary pulmonary hypertension, wherein the secondary pulmonary hypertension is secondary to congestive heart failure, chronic hypoxic lung disorder, inflammatory or collagen vascular disease, congenital heart disease associated with left to right shunting, and pulmonary thromboembolism; and wherein the pulmonary vascular resistance is lowered to a greater extent than the systemic vascular resistance.
