PROBLEM TO BE SOLVED: To improve the productivity of an optical waveguide device in which an optical fiber array mounted with an optical fiber is connected to a waveguide with an adhesive by reducing the processing time of the end face of the waveguide, to improve the connection strength of the optical fiber and the array, and to provide a method of manufacturing thereof. SOLUTION: An adhesive is applied on end faces 17A and 17B of the optical waveguide device 17 on which an end face 34A having an inclination angle -θ1 and an end face 34B having an inclination angle +θ2 are formed by grinding a waveguide 21 after forming notched parts 33A and 33B on the end faces 32A and 32B of a substrate 18 by a grinding process, and an optical fiber array is connected.
