PROBLEM TO BE SOLVED: To provide a form for a flat plate-shaped concrete block into which a reinforcing rod can be smoothly inserted with a side plate being closed and which does not cause a damage on the flat plate-shaped concrete block at the time of demolding, and a molding method of the flat plate-shaped concrete block. SOLUTION: The form is used for molding the flat plate-shaped concrete block 15 in which a reinforcing rod 14 comprising vertical rods 14a and horizontal rods 14b arranged in parallel crosses is embedded and constituted of a C-shaped side frame body 13, a front frame body and a rear frame body, and the side frame body 13 is constituted of a bottom plate 13a and a pair of side panels 13b pivotally mounted on the bottom plate 13a. A protruded part 13d for forming the central recessed part 15a of the flat plate-shaped cement block 15 is formed to the side panel 13b and horizontal rod receiving metal fittings 16 for holding a horizontal rod cap 18B are provided above the protruded part 13d while vertical rod receiving metal fittings 17 for holding a vertical rod cap 18A are provided to the bottom plate 13a.
