<P>PROBLEM TO BE SOLVED: To reduce the work burden of setting modifications accompanying change of a form of paper or the like having entry columns where character information should be written, when changing the form of the paper. <P>SOLUTION: An information reader comprises a scanner 3 and a PC 4 and has a character information read means, a discrimination information read means, a data generation means, and a data output means. The character information read means is capable of reading character information written in an entry column 8. The discrimination information read means is capable of reading a two-dimensional code 9 which is recorded in at least a part of a read object area including the entry column 8 and its peripheral area 10 and shows contents represented by character information written in the entry column 8. The data generation means generates character data and discrimination data on the basis of character information and the two-dimensional code 9 which are read by the character information read means and the discrimination information read means.
