PROBLEM TO BE SOLVED: To provide a sheet for an envelope type slip such as an envelope type pay slip which makes smooth a feeding operation in a printer and also a folding operation in a folding part. SOLUTION: The sheet 1 for the envelope type pay slip has a constitution wherein a lower-position sheet having an adhesive provided on the surface entirely at the three peripheral edges and partially in the other part is superposed on and adhered to the back side of an upper-position sheet 2 prepared by providing a release agent entirely on the back side. A perforation 6 for folding the front side of the upper-position sheet 2 as a superposing surface is provided, and perforations 7, 8 and 9 for opening are provided corresponding to the adhesive coated portions of the three peripheral edges. An address information entry part 10 is provided on the front side of the upper-position sheet 2 at a position where this part is exposed when folding is made along the folding perforation 6, and a pay amount entry part 11 and a deduction amount entry part 12 are provided at a position to be hidden, while a slit 13 for peeling so as to expose the adhesive on the lower-position sheet is provided.
