PROBLEM TO BE SOLVED: To form an image of high quality which is free of scatter of a toner image even when a polymerizable toner is used. SOLUTION: A fixing device 60 which fixes a toner image carried on a form P has a fixing roll 61, an endless belt 62 which can move in contact with the fixing roll 61 while forming a nip part N, and a fixation entrance guide 56 which guides the form P to the nip part N, and the fixation entrance guide 56 has an upstream-side guide part 562 which guides the form P to an area where heat which is much enough to mutually fuse toner images is conducted while holding a state where the toner image is electrostatically restrained to the form P and a downstream-side guide part 561 which guides the form P to vicinity of the nip part N in a state where the toner images are mutually fused.
