PROBLEM TO BE SOLVED: To facilitate aligning work and also to suppress occurrence of secular characteristic degradation as to an optical waveguide module for connecting optical fibers and an optical waveguide optically using a fiber array. SOLUTION: In a module equipped with a fiber array 11 on which optical fibers 20 are fixed and a PLC (Planar light wave circuit) board 12 on which a waveguide 21 which is connected optically to the optical fibers 20 arranged on the fiber array 11 is formed, microlenses 14 are arranged among the optical fibers 20 and the PLC board 12 and the microlenses 14 are integrally formed on the fiber array 11.
