PROBLEM TO BE SOLVED: To provide a delivery slip in which a receipt part can be separated from a slip substrate and which is excellent in appearance after the separation of the receipt part. SOLUTION: The delivery slip 1 has a delivery part 5 and a receipt part 7 arranged in parallel on the slip substrate 2. The receipt part 7 is sectioned and formed by a tearing perforation 6 to be separated therefrom. An adhesive agent 3 is applied over the rear face of the slip substrate 2 except the part corresponding to the receipt part 7, and is covered with a protective paper 4. The protective paper 4 has a remaining part 17 one size larger than the receipt part 7 corresponding to it and having its remaining part 17 sectioned and formed by a slit 18 so as to have the adhesive agent 3 on the periphery, and has a releasing agent to allow the protective paper to be released except the remaining part 17. Advertising information 20 is displayed on the surface facing the receipt part 7 of the remaining part 17, and is exposed visually when the receipt part 7 is separated therefrom.
