ORDERING POSTCARD FORM WITH DUPLICATE

PROBLEM TO BE SOLVED: To provide an ordering postcard form with a duplicate which can readily leave a duplicate on ordering a commodity. SOLUTION: The ordering postcard form 1 with a duplicate has an ordering postcard 3 and the duplicate 4 connected with each other via a bending and tearing perforation 2. The rear face of the ordering postcard 3 is used as address displaying surface, and the order information entry surface on the front surface has an ordered article information entry column 16 and an orderer information entry column 17. The rear face side of the duplicate 4 has an ordered article information entry duplicate column at a position corresponding to the ordered article information entry column 16 when the duplicate 4 is folded on the ordered article information entry surface of the ordering postcard 3 at the bending and tearing perforation 2. The ordering postcard form has furthermore a copying structure 22 to provide a duplicate of the same content in the ordered article information entry column 16 when an order is written on the ordered article information entry column under the folding state.
