PROBLEM TO BE SOLVED: To provide a delivery form which can prevent a delivery contractor from taking out an addressee's copy slip by mistake, and also, makes easier to take out required delivery slips in the delivery process regarding the delivery form equipped with a pasting slip side, the addressee's copy slip, and delivery slips. SOLUTION: This delivery form has a bag-like section which is constituted by bonding an upper side paper being the pasting slip (11) and a lower side paper being confronted with the upper side paper to form a one-edge opened bag-shape. In the bag-like section, at least the addressee's copy slip (12) and the delivery slip (13) are removably provided in order from the pasting slip (11) side, and information which is recorded on the pasting slip (11) is selectively duplicated on the addressee's copy slip (12) and the delivery slip (13). In the delivery form, the length of the addressee's copy slip (12) is made approximately equal to or shorter than the length to the opening (4) of the upper side paper. Also, the length of the delivery slip (13) is made to protrude to the outside of the opening (4) of the upper side paper to solve the above mentioned problem. COPYRIGHT: (C)2005,JPO&NCIPI
