PROBLEM TO BE SOLVED: To arbitrarily provide a fitting embodiment by turning reversely the drawing direction of a flexible flat cable 40 to a piezoelectric actuator 12. SOLUTION: On a wide width face of a top sheet 35 of the piezoelectric actuator 12, a number of individual surface electrodes 58 electrically conducted to an individual electrode, and a number of common surface electrodes 57 electrically conducted to a common electrode, are formed in an island shape. The common surface electrodes 57 and the individual surface electrodes 58 are arranged in a point symmetry using a central point on the wide width face of the top sheet 35 as a center O of symmetry, and are arranged in a point symmetry using an individual connecting electrode part 78 formed on the wide width face of the flexible flat cable 40 as the center O of symmetry. The common connecting electrode part 77 is composed of the first parts 77a and 77a along a pair of long side edges, and the second parts 77b connecting to this and along a free end edge, and can be electrically connected respectively with a number of the common surface electrodes 57 and the individual surface electrodes 58 when the flexible flat cable 40 is rotatably moved by 180°around the center O of symmetry.
