<P>PROBLEM TO BE SOLVED: To prevent an error state by providing an image forming apparatus with a report function of diagnosis information relating to its own operating condition. <P>SOLUTION: The image forming apparatus capable of forming an image on a recording medium, such as paper is equipped with an operating condition inspection/diagnosis means step S 20 which inspects and diagnoses the operating condition of the image forming apparatus and a diagnosis information output means step S 40 which outputs the diagnosis information including the result of the inspection of the operating condition by image formation to the medium at a predetermined output timing or the output timing meeting an output request.
