A portable mechanical insulin injection device is provided to inject meal type insulin and basic type insulin through change of connecting tube separately. In the portable mechanical insulin injection device, an injection needle in an insulin injection unit is injected to a human body. A gear operation manual rotating part(106) is more precisely rotated through a gear apparatus(105) in an insulin injection part of a connecting tube when injecting insulin. The ultraprecise amount of insulin in the insulin needle is ejected by movement of a plunger through movement of fixed nut for moving the plunger(104) and the ultraprecise amount of insulin is ejected to the insulin injecting unit to inject insulin to the human body.
