<P>PROBLEM TO BE SOLVED: To prevent a swelling of a part to which an information recording medium is stuck, of a printed matter. <P>SOLUTION: An image forming apparatus comprises a print mechanism 1 for printing a job sheet 3, a supply mechanism 2 connected to a paper feed port 17 through which the job sheet is supplied to the print mechanism from the outside, a cut mechanism 4 for cutting and removing a part of the job sheet ejected from the print mechanism, a booklet mechanism 5 for forming the job sheet sent from the print mechanism into a prescribed shape, and a sticking mechanism 7 for sticking an information recording medium 6 to at least a part of the job sheet, and these print mechanism, the supply mechanism, the cut mechanism, the booklet mechanism, and the sticking mechanism are arranged in series. <P>COPYRIGHT: (C)2005,JPO&NCIPI |
