PROBLEM TO BE SOLVED: To provide a person of advanced age suffering from obstacle with a convenience for use by the way of forming individual columns consisting of 5, 6, 7, 8, 9, 10, 11 and 12 in a frame column 4 provided in a single space 1, and of classifying these columns consisting of 5, 6, 7, 8, 9, 10, 11 and 12, and of attaching a corner seal 14. SOLUTION: The frame column 4 with 7 width-trains is formed by intersecting a vertical jump-off line 2 with a horizontal jump-off line 3 on the single space 1. The frame column 4 is provided with the column 5 for day and good omen for the day, the column 7 for filling in a weather and physical condition and the blank column 11 for filling in. Each column is made colored with a different color for enabling the written characters to be visibly confirmed, and classified by color. On the space 1, an indication 13 calling attention is marked using a phosphorescent coating 4. The corner seal 14 with a chip 14b is provided in a lower right edge section of the space 1.
