PROBLEM TO BE SOLVED: To provide an inexpensive fine particle conveying device whose structure can be simplified and an image forming apparatus. SOLUTION: This fine particle conveying device has a plurality of electrodes 12 for the purpose of generating an electric field in which a conveyance substrate 131 conveys a toner as fine particles in a liquid, wherein driving waveform voltage Pv is applied from a driving circuit 2 to the electrodes 12. The applied driving waveform voltage consists of three waveform voltages Va, Vb, Vc different from one another in phase. The electrodes 12 are divided into a group of electrodes 12a to which the waveform voltage Va is applied, a group of electrodes 12b to which the waveform voltage Vb is applied, and a group of electrodes 12c to which the waveform voltage Vc is applied. The mutually different groups of electrodes are arranged so that these are made mutually adjacent, and a phase shift electric field (traveling wave electric field) is generated among the adjacent electrodes 12a, 12b, 12c to convey the toner.
