PROBLEM TO BE SOLVED: To provide a form which cuts a medium cost (especially, a material cost) to the maximum possible extent while securing a specified human interface and ensures outstanding productivity. SOLUTION: The concealment card 10 is formed of a main body of postcard 11 (a first sheet) with an application entry column 11a in which information can be entered; a concealable piece 13 (a second sheet) which is used concurrently as protecting paper with a rereleasable adhesive layer 12 which is formed on at least a part of the laminating surface of the conceable piece 13 with the main body 11 of the postcard 11 and can be laminated releasably; and an unreleasable superadhesive part 15 which is formed on at least a part of the laminating surface of the concealable piece 13 with the main body 11.
