PROBLEM TO BE SOLVED: To provide an X-ray inspection device capable of stably inspecting missing items by reducing the influence on the conveyance direction due to displacement. SOLUTION: A mask region setting means 15a sets a mask region for detecting missing items compatible with the contents of an object to be inspected. A reference position calculating means 19 compares the X-ray transmission image of the contents obtained, when the object to be inspected is exposed to X-rays, with a reference position detection limit value preset according to the arrangement of the contents, and discriminates the position of the X-ray transmission image of the contents exceeding the reference position detecting limit value first, as nearly the tip position of the contents positioned at the top of a housing body for calculating as a reference position. A missing item discriminating means 20a overlaps the X-ray transmission image of the contents to the set mask region for missing item detection, for discriminating the presence or absence of the missing item of the contents with the reference position calculated by the reference position calculation means set as a reference.
