PROBLEM TO BE SOLVED: To provide an X-ray diagnostic instrument for adjusting alignment in a short time without nonuniformity by worker. SOLUTION: The instrument includes a converting means for converting an X-ray radiated from an X-ray tube and transmitted through a subject into an optical image; a means for forming an X-ray fluoroscopic image from the optical image via an optical system; a display means for displaying the X-ray fluoroscopic image on a monitor; a reference region designating means for displaying a reference point indicating the center position of the monitor, a reference circle having a fixed radius with the reference point as a center, and a marker indicating a position for obtaining the maximum luminance in the optical image; a peripheral edge information detecting means for detecting the position where the luminance of the optical image becomes the maximum value; and a comparison arithmetic means for calculating a deviation amount between the reference point and the position with the maximum luminance, and displaying the result in the display means.
