PROBLEM TO BE SOLVED: To solve such a problem that as a connecting substrate connecting each driving coil is attached to a side plane of a lens holder, the weight of the lens holder is unbalanced, and rolling is easy to occur. SOLUTION: A pair of driving coils 8, 9 attached to the lens holder 4 are provided so as to oppose respectively to a pair of magnets 5, 6 arranged at both sides holding the lens holder 4 between them, and the connecting substrate 18 connecting each driving coils 8, 9 is installed at a bottom plane of a reverse side to an attached plane of an objective lens 7 of the lens holder 4. Thereby, connection of each driving coil 8, 9 is performed, while the centroid of the lens holder 4 deviated to the attachment plane side of the objective lens 7 is made close to the center in a state in which the connection substrate 18 is not installed.
