The tongue of the cushion is received in the gap in the shell to secure the cushion to the shell. The inner wall (62) of the cushion is discontinuous in a nasal bridge region of the cushion. A forehead strap assembly (30a) for connection with the shell for helping to hold the shell on a user's head includes a pin (188) and a forehead adjuster (140a) connected between the shell and the forehead strap (190).</p>
