In another embodiment, the card comprises first plastic wallet-sized substrate having emergency warning indicia and a second foldable paper substation having indicia disposed on the substrate for requesting information and designating a printing area for identifying an owner of the animal, an address of the owner, a contact person, veterinarian information, and a description of the animal. Kits include the card along with a sticker having emergency warning indicia corresponding to the card. The sticker is attachable to a building for locating the animal.
