<P>PROBLEM TO BE SOLVED: To allow an information collection source to collect information from an unspecified large number of consumers without taking trouble. <P>SOLUTION: This information collection system is provided with: an information collection means 40 for collecting the information; a ticket 10 recorded with a two-dimensional code 11 including an electronic mail address of the information collection means 40 together with an identifier indicating that it is destination information, and including questionnaire information for collecting the information; and a portable terminal 20 connectable to a communication line 30, capable of reading the two-dimensional code 11 recorded in the ticket 10, and setting the electronic mail address of the information collection means 40 as a destination, and editing and displaying an electronic mail transmission screen placed with the questionnaire information when the identifier is recognized in the two-dimensional code 11 read from the ticket 10. <P>COPYRIGHT: (C)2005,JPO&NCIPI |
