FIELD: medicine, surgery. ^ SUBSTANCE: one should dissect radically a giant bleeding cardiac ulcer, restore the integrity, natural shape and reservoir gastric function due to longitudinal-cross-sectional gastroplasty, replace the edges of gastric defect from the side of its posterior wall, perform T-shaped suture, restore the lesser curvature at the distance against cardiac bagasse being above 0.5 cm with longitudinal suture along the middle line of the lesser curvature; at the distance being below 0.5 cm one should shift longitudinal suture left-hand and upwards onto anterior wall of esophago-gastric passage. One should form the posterior gastric wall in cross-sectional direction. The method enables to keep all gastric departments and its innervation, and save the mechanism of evacuation out of the stomach. ^ EFFECT: higher efficiency. ^ 3 cl, 2 dwg, 2 ex |
