PROBLEM TO BE SOLVED: To provide a concealing label sheet which facilitates trader's work when collecting applications and allows confirmation of skimming of the concealed information. SOLUTION: This concealing label sheet 10 comprises: an information fill-in piece 11 having an information writing region 11a; a concealing piece 12 continued to one side of the fill-in piece 11 and having a concealing region 12a for concealing at least the fill-in region 11a; a re-peelable and re-stickable adhesive 13 applied to the concealing piece 12 outside the concealing region 12a; a strong adhesive 14 applied to an end part of the concealing piece 12; a protective sheet 15 integrally protecting the adhesives 13 and 14; an opening perforation 16 formed in three sides of the concealing region 12a (in a U-shape) for opening the sheet; and a notch 17 for facilitating peeling, etc. COPYRIGHT: (C)2005,JPO&NCIPI
