<P>PROBLEM TO BE SOLVED: To streamline data input and correction even outside a medical institution and ensure image reading allowing recombination by masking information for personal identification and imaging only a medical attribute information description part when a medical record is read with an image scanner. <P>SOLUTION: Digital data read from a medical record are masked in a specified data part for personal identification, and the remaining medical attribute information part is captured and saved as image data.
