<P>PROBLEM TO BE SOLVED: To provide a device for creating a medical interview sheet for simply creating a medical interview sheet for a medical chart by an operation of outpatient itself when the outpatient is accepted at a hospital. <P>SOLUTION: A portable input means of the device for creating the medical interview sheet has a function for acquiring information of a scanner with an medical interview sheet, in which a patient has already filled up, is read therein and comprises the steps of registering the information about the patient in a patient table as a receipt factor; and registering image data on the facial picture of the patient imaged by a facial picture imaging means in association with the patient's name registered in the table into a patient image file. A portable input means has a medial interview sheet creating means for creating basic data about the medical interview sheet, based on an ID number for a patient registration card of the patient associated with the table, and has a step that the patient and/or the person in charge of the hospital output the basic data about the patient to a printer terminal after confirming the basic data, on the spot, and print out the medical interview sheet for the medical chart from the printer terminal. <P>COPYRIGHT: (C)2005,JPO&NCIPI |
