The insert through hole (106-112) is aligned with the plate through hole (28, 32). The cam surface (118, 120, 122, 124) contacts the cleat (14) when the plate (24, 26) interfaces with the cleat (14). The spacer (70, 72) includes a spacer through hole (90, 92). The spacer (70, 72) is located adjacent to the insert (74, 78), and the spacer through hole (90, 92) is aligned both with the plate through hole (28, 32). and the insert through hole (106-112). The coupler is introduced through the plate through hole (28, 32), insert through hole (106-112) , and spacer through hole (90, 92).
