PROBLEM TO BE SOLVED: To provide a pharmaceutical composition comprising a phosphodiesterase IV inhibitor (PDE-IV inhibitor) and an anticholinergic agent which is useful for treating a respiratory disease and the like. SOLUTION: The pharmaceutical composition comprises 7-[2-(3,5-dichloro-4-pyridyl)-1-oxoethyl]-4-methoxy-spiro[1,3-benzodioxole-2,1'-cyclopentane] or its pharmacologically admissible salt and an anticholinergic agent. A therapeutic agent for chronic obstructive pulmonary disease (COPD) or asthma is composed of 7-[2-(3,5-dichloro-4-pyridyl)-1-oxoethyl]-4-methoxy-spiro[1,3-benzodioxole-2,1'-cyclopentane] or its pharmacologically admissible salt and (b) an anticholinergic agent and is administered simultaneously or separately with an interval of time.
